Pre-Adoption Checklist

  1. All work is done by a limited number of very busy volunteers, so please be patient as we work to review the application as quickly as possible.  Generally all application interviews and processing will be completed after during the week or on the weekend.                                                  
  2. Complete this application in its entirety so it may be processed.

  3. All current or previous pet information (in the last 10 years) MUST be disclosed on the application.  Failure to provide this information may result in the denial of your application.

  4. All current pets and previous pets (in the last 10 years) must be up to date on vaccinations/rabies and spayed/neutered prior to adopting (exceptions may be made for puppies/kittens or animals with special medical needs).  We will conduct a vet reference to confirm that your animals(s) were/are fully vetted so please provide your current/previous Veterinarian information.  Failure to provide this information may result in the denial of your application.

  5. PCHS does not adopt animals that will live outside unless the animal is approved to live outside and a home visit in conducted to see what the animals living conditions will be.  If you are unsure if the animal you are applying for is approved for outside living please go to our home page, click on the “Adopt a Dog/Cat” tab and read the petfinder.com animal information page, it will be listed.

  6. If you rent/lease your place of residence, you must obtain permission from your landlord prior to applying for adoption.  We are required by the State of Illinois to verify that you have received permission so please include your landlords contact info on application.  Failure to provide this information may result in the denial of your application.

  7. In some cases, when adopting certain breeds (Pit Bulls, Rottweiler’s, etc) or special needs dogs (or cats), we would require a home visit before the dog/cat leaves our facility.  We may also do an after-the-fact home visit to make sure that all needs are being met.  Please be aware of this before filling out the application.

If you do not meet any of these requirements and/or have questions, please contact pchsinfo@yahoo.com prior to completing an adoption application.  We understand that every adoption is not black and white and will work with you if we are able to.

***Additional Comments Regarding Adoptions:  Adoptions are conducted on a first come first serve basis meaning the first application submitted will be processed first.  If that application is approved they will be granted the adoption.  If that application is denied the next application in line will be taken into consideration.  Submission of your application does not mean the application is approved.

Also, we strive to find the best fit not only for adopters but for our animals.  We reserve the right to deny applications if we do not feel that the animal you have applied for would be a good match for your family.  This does not mean that you would not be eligible for another animal that would be a better match for you.  Keep looking!

Adoption Fees - What's included?

When you adopt a Perry County Humane Society animal, your pet will come to you with complete medical services given depending on the age of the animal.

The adoption fees cover the following medical costs:

Dogs: spay/neuter, vaccinations including rabies and bordetella, deworming, heartworm test, microchip, heartworm preventative, and flea and tick preventative.

------- Adoption fee is $150.00 for dogs/puppies.

Cats: spay/neuter, vaccinations including rabies, FIV/FELV test, and frontline.

-------Adoption fee is $100.00 per cat or $75 for up to two kittens.

**Seniors for Seniors Program**

PCHS is also happy to announce our *Seniors for Seniors* program.  If you are 55 + yrs old  & looking to spend your golden years with a Senior pooch or cat your adoption fee will come at a discounted adoption price.  Please mention that you are interested in the Senior for Seniors program when filling out the adoption application.
